To make it easier for you to refer clients to us, we have prepared this page with links to the required documents which must be completed.
You are welcome to print these off, assist the applicant to complete them and then either
- fax the documents with the appropriate applicant's evidence via 07 5502 6662 OR
- mail the completed documents with eligibility evidence to

Please note that these are the latest versions of the DEEWR documents and must be used from 1 February 2009.
Application Forms
Confirmation of Enrolment Form
- All pages must be returned with the application, completed where appropriate with:
- all required elibility evidence
- an original or certified copy of the complete letter from Centrelink advising of benefit paid, amount, date and which states the applicant's name and current address
- information about the relevant course for Section 8 (last page) can be found on:

- our TAA04 Training and Assessment prospectus
Enrolment Forms
Your applicant will also need to complete the appropriate enrolment form. All sections must be completed excluding Fees Remittance (as the enrolment will hopefully be paid for through the government funded Productivity Places Program) but INCLUDING the Declaration beneath the Remittance:
- BSB30207 Certificate III in Customer Contact
- HLT32507 Certificate III in Health Services Assistant
- TAA40104 Certificate IV in Training and Assessment
In addition, your applicant must have a copy of DEEWR's Your rights and responsibilities brochure.
